Job summary

Hanger, Inc. is seeking a Community Care Coordinator to deliver exceptional patient support and advocacy for prosthetic and orthotic care. You will coordinate care steps, maintain relationships with referral partners, and guide patients through pre- and post-operative processes.

The role emphasizes communication, documentation, and collaboration with clinicians to ensure a seamless patient experience across Hanger Clinic locations.

Qualifications

• Minimum 2 years of experience in a patient care environment.
• BA/BS in Health related field or equivalent work experience.
• Ability to be credentialed in major hospital systems.
• Experience in providing a high level of customer service in a fast paced environment, including handling confidential data with discretion.
• Must have, or be eligible to obtain, a valid driver’s license and driving record within the stated policy.

Responsibilities

• Contact new patients before or after initial consult and enter them into the clinic’s patient management system.
• Support patients with amputations with information, resources, and counseling throughout the process.
• Coordinate AMPOWER visits and build referral relationships for Hanger Clinic.
• Assist with obtaining clinical documentation for insurance coverage.
• Engage patients with follow-up communications and appointments to ensure quality care.
• Provide updates on follow-up visits with surgeons and care plan changes as needed.
• Complete hospital or rehab facility visits when required by the care team.
• Identify and follow up on referrals to ensure continuity of care.
• Serve as a liaison between patients, physicians, and clinic staff.
• Utilize CCC tools to maintain communication with patients and health professionals.
• Assist with upcoming visits and fittings; pursue ABC Orthotic Fitter credential if applicable.
