Responsibilities

• Cares for patients as part of a care management program.
• Manages patient caseloads through in-person, telephonic, and electronic communications.
• Conducts outreach calls to encourage program participation.
• Assists patients with self-care needs and coaches to meet goals.
• Helps schedule provider visits, arrange transportation, and prepare visit agendas.
• Educates patients about preventive monitoring and caregiver plans.
• Documents activities and supports care team productivity.
• Collaborates with the multidisciplinary clinical care team; evaluates processes and suggests improvements.
