Job summary

JWCH Institute, Inc. in Commerce, CA, is seeking a full-time Case Manager-Corporate to deliver comprehensive case management for patients served by JWCH, Wesley Health Centers, and Kaiser ED referrals.

The role involves coordinating medical and social services, developing care plans, and advocating for patients while maintaining accurate records. The ideal candidate has relevant social work education and experience, strong assessment and planning skills, and comfort working in low-income urban

Qualifications

• AA or BA in Social Work, Sociology, or related field with at least 1 year care coordination experience.
• High School Diploma or GED with 3-4 years care coordination experience.
• Willing to work in low-income areas of Los Angeles with persons experiencing mental illness and homelessness.
• Strong assessment and care planning skills; excellent organizational and time-management abilities.
• Experience with Electronic Health Recording (EHR), Next-Gen; bilingual English/Spanish preferred.
• DMH funded program experience preferred.

Responsibilities

• Conducts comprehensive case management: intake, assessment, care plan development, referrals, and follow-up.
• Monitor and adjust care plans to optimize patient outcomes.
• Connect clients to medical, housing, transportation, and other support services.
• Ensure timely follow-up on referrals and appointments.
• Collaborate with multidisciplinary team to address needs of patients with mental illness or homelessness.
• Advocate using motivational interviewing and trauma-informed care.
• Coordinate medical and psychosocial services addressing social, emotional, and practical needs.
• Educate patients and families on available resources and support services.
• Maintain accurate patient records, including assessments, care plans, and progress notes.
• Prepare and submit required reports and documentation in a timely manner.
• Participate in team meetings, case conferences, and other collaborative activities.
• Adhere to safety, compliance, and regulatory requirements.
