Job summary

Wesley Health Centers - JWCH Institute, Inc. in Los Angeles is seeking a Case Manager to provide comprehensive case management to eligible patients, including housing insecure adults with complex health and behavioral health needs.

You will assess needs, develop tailored care plans, coordinate resources, and advocate for optimal outcomes with a multidisciplinary team. Proficiency in EHR, strong communication, and cultural competence are essential.

Qualifications

• Medical Assistant certification preferred, with at least one year of case management experience.
• High school diploma or GED required.
• Minimum of two years of experience as a care coordinator, community health worker, or in a similar role.
• Experience working with individuals with chronic conditions, homelessness, or frequent ER visits.
• Proficiency in electronic health records (EHR) and data entry.
• Strong computer skills and communication abilities.
• Bilingual English/Spanish preferred.

Responsibilities

• Collaborate with providers, care plans, and housing/navigation teams to coordinate care.
• Perform comprehensive case management including intake, assessment, and plan implementation.
• Connect members with essential services, arranging referrals for care, transportation, and housing.
• Accompany patients to appointments and advocate for their needs.
• Utilize trauma‑informed care, motivational interviewing, and harm‑reduction strategies.
• Coordinate discharge planning with clinical staff and monitor treatment adherence.
• Collect data for assessments and participate in case conferences and reporting.
• Maintain confidentiality and participate in safety/compliance trainings.
