Job summary

JWCH Institute, Inc. is seeking a Case Manager-Corporate to deliver comprehensive case management for patients across JWCH and Wesley Health Centers, coordinating care plans, referrals, and advocacy. You will collaborate with healthcare providers, social services, and community resources to improve outcomes and ensure timely access to services.

Qualifications

• AA or Bachelor’s degree in Social Work or related field is required/preferred.
• High School Diploma or GED with 3-4 years of experience in relevant role.

Responsibilities

• Provide comprehensive case management services from intake to follow-up.
• Coordinate care plans, referrals, and follow-up actions with multidisciplinary teams.
• Advocate for patients using motivational interviewing and trauma-informed care techniques.
• Coordinate a range of medical and psychosocial services addressing social, emotional, and practical needs.
• Maintain accurate patient records and timely reports in DMH-contract compliant manner.
• Engage in outreach activities and program initiatives within and outside clinic.
• Ensure safety, infection control, and regulatory compliance in daily work.
