Job summary

Wesley Health Centers - JWCH Institute, Inc. in East Hollywood, CA seeks a Case Manager to provide comprehensive case management for patients within JWCH, Wesley Health Centers, and Kaiser ED referrals.

You will assess needs, develop care plans, coordinate services, and advocate to improve outcomes. Responsibilities include coordinating medical, housing, and psychosocial referrals, maintaining records, and working with a multidisciplinary team.

Qualifications

• AA or BA in Social Work, Sociology, or related field, with 1+ year in care coordination preferred
• High School Diploma or GED with 3–4 years experience as care coordinator, housing navigator, or similar role
• Willing to work in low-income areas of Los Angeles with people experiencing mental illness and homelessness
• Bilingual English/Spanish preferred
• Experience with DMH funded programs preferred
• Experience with EHR/Next-Gen systems preferred

Responsibilities

• Provide comprehensive case management including intake, assessment, and care plan development
• Coordinate referrals to medical, addiction, housing, transportation, and other services
• Monitor and adjust care plans to improve outcomes
• Advocate for patients using motivational interviewing and trauma-informed care
• Maintain patient records and documentation for DMH contract standards
• Collaborate with multidisciplinary team and participate in meetings
• Coordinate outreach activities and ensure timely follow-ups
