Job summary

Qualifications

• AA or Bachelor’s degree in Social Work, Sociology, or a related field
• High School Diploma or GED with 3-4 years of related experience
• Willing to work in Los Angeles low-income areas with homeless or at-risk individuals
• Experience with EHR, Next-Gen preferred

Responsibilities

• Provide comprehensive case management services including intake, assessment, and care planning
• Coordinate medical, psychosocial, housing, transportation, and other referrals
• Advocate for patients using motivational interviewing and trauma-informed care
• Maintain accurate patient records and submit required reports
• Participate in team meetings, case conferences, and safety/compliance trainings
