Responsibilities

• Coordinate and provide care with offsite support to be client-centered and efficient.
• Maintain caseload and productivity through regular client contact to monitor progress.
• Conduct virtual and in-person meetings with clients.
• Draft service plans and review progress, determining case closures with growing independence.
• Provide ongoing services including crisis intervention, advocacy, transportation assistance, budgeting, housing assistance, and referrals.
