Indiana University Health is seeking team-oriented staff to provide outreach, engagement, education, advocacy, and navigation services to patients and families in the Ball Memorial area. The role emphasizes addressing social determinants of health and coordinating care across disciplines to improve outcomes.

You will connect patients with prenatal, postpartum, primary, specialty, and behavioral health services, while educating on self-management and RPM programs.

Qualifikationen

• Requires 0-3 years of relevant experience.
• Must obtain CHW certification within 1 year of hire.
• Basic computer skills and knowledge of local resources.

Aufgaben

• Provide outreach, engagement, education, advocacy, and navigation services to patients and families.
• Assess barriers to care and conduct SDOH screenings to identify needs.
• Connect patients with healthcare services and community resources; facilitate referrals.
• Support care coordination activities, including appointment scheduling and follow-up outreach.
• Assist patients in accessing maternal health, infant care, chronic disease management services.
• Identify eligible patients and facilitate RPM program enrollment; educate on equipment use.
