Job summary

Neighborhood Health Center is seeking a care coordinator to support Buffalo patients by coordinating care with internal teams and community resources. You will listen with kindness, help navigate services, and assist families with referrals and authorizations to close care gaps.

You’ll schedule appointments, advocate for patients with community agencies, collaborate with nursing staff, and educate patients to improve health outcomes while supporting health equity.

Qualifications

• High school diploma or equivalent required, associate degree preferred.
• Experience in community health, patient resource coordination or equivalent.
• Excellent written and verbal communication, problem solving skills.
• Able to read, write and speak English; Spanish preferred.

Responsibilities

• Coordinate care and patient navigation with internal providers and community resources.
• Assist patients with referrals, prior authorizations, and tracking referrals to completion.
• Schedule appointments and advocate for patients with community agencies.
• Consult with providers regarding patient needs for referrals or linkages.
• Collaborate with nursing staff, billing, and other departments on patient needs.
• Provide education to improve health and wellbeing.
