Job summary

Community Healthcare Network is seeking a Patient Navigator to support the Care Management team in providing services to a diverse patient population in New York City. The navigator will assist with locating patients, linking them to care, and maintaining accurate records in the EMR.

The role includes conducting outreach to patients, educating them on health conditions, scheduling appointments, and ensuring retention in primary care.

Qualifications

• High school diploma or GED required; two years office and/or related experience preferred.
• Ability to assist in scheduling and coordinating patient care.
• Strong communication and organizational skills are essential.

Responsibilities

• Conduct new patient assessments and update roster information.
• Escort patients to appointments and educate them on health conditions.
• Provide appointment reminders and maintain case records in the EMR.
• Assist Care Managers with locating patients and linking to care.
• Maintain accurate documentation, including progress notes and care plans.
• Support the Care Management team in coordinating services for the caseload.
