Job summary

Chicago Family Health Centers is seeking a dedicated Care Coordinator to guide patients through care plans, ensure follow-up, and promote healthy lifestyles. You will serve as a vital link between patients and the healthcare team, assisting in referrals and coordinating care during hospital discharges and ER visits.

The role emphasizes patient advocacy, utilization management, and timely follow-ups, with responsibilities spanning inpatient/outpatient care and population-based management.

Qualifications

• Associates Degree or 3+ years health-related experience.
• Proficient in EHR, MHN Connect, Word and Excel.
• 3+ years of healthcare experience.

Responsibilities

• Act as peer support for enrolled patients and advocate as they navigate the medical system.
• Manage inpatient and outpatient utilization and medical care follow-up.
• Perform population-based management activities.
• Ensure health risk assessments are completed and shared with relevant entities.
• Report health follow-up and coordinate with the care team.
• Distribute disease-specific education to patient panels and community.
• Provide reminder calls for appointments and follow-ups.
• Assist patients with specialty care, PCP visits, and labs/diagnostics.
• Manage and track medical referrals.
