WVUH West Virginia University Hospitals is seeking a Patient Navigator to guide patients through the healthcare system. You will collaborate with a multidisciplinary team to manage patient pathways, verify benefits, and coordinate care across services.

The role requires strong communication, payor knowledge, and the ability to navigate complex healthcare resources to reduce barriers to care.

Competenze

• Bachelor’s degree in Healthcare Management, Public Health, or related field required or equivalent experience.
• Associate’s degree plus two years of directly related healthcare experience also qualifies.
• High school diploma plus four years of related experience acceptable.

Mansioni

• Establishes close relationships with patients and serves as primary contact.
• Analyzes payor population by collecting market data and verifies insurance benefits for obesity treatment.
• Maintains knowledge of payor pre-certification requirements for meds, tests, surgery, referrals.
• Educates providers and staff on payor regulations to prevent denials.
• Identifies resources to remove barriers to care and helps navigate healthcare services.
• Coordinates with service lines to ensure appointment coordination.
• Schedules patient appointments and coordinates labs or tests, including reminders.
• Monitors admitted patients and rounds daily; assists with discharge scheduling.
• Assists with obtaining prescriptions and assesses cost and availability.
• Documents patient information and retrieves outside records as needed.
• Engages family or community resources for emotional, social, and financial needs.
• Collaborates with social services for resource and cost management and referrals.
• Performs other department specific duties as assigned.
