• Assists patients with access to insurance and social service programs.
• Provides reminders of upcoming appointments and acts as liaison between patients and providers.
• Identifies and resolves barriers to care.
• Educates and counsels patients on health and life issues.
• Maintains knowledge of community health resources and programs.
