Job summary

The University of Miami Health System seeks a Patient Navigator to support patients through their care journey in South Florida. You will assess patient needs, coordinate referrals, and connect patients with insurance and social services to ensure timely access to care.

Qualifications

• High School Diploma or equivalent required.
• Minimum 4 years of relevant experience required.
• Certification or license may be required depending on department.

Responsibilities

• Maintains knowledge of medical symptoms to identify type and urgency of medical need.
• Conducts patient evaluations for effective treatment planning.
• Helps patients access insurance and social services they are eligible for.
• Provides patient reminders and serves as liaison between patients and providers.
• Explains appointment and payment policies (parking, cash policies, charges, ancillary services, cancellations).
• Identifies and resolves barriers to care and advocates for patients.
• Intervenes as liaison/advocate for patients, physicians, and staff to facilitate care.
• Educates and counsels patients on health and life issue considerations.
• Maintains knowledge of community health resources and programs available to patients.
• Assists in building partnerships with local community physicians and referral sources.
