UCLA Health in Los Angeles is seeking a patient services liaison to support physicians and patients in the Brentwood office. You will act as a key link between patients and providers, helping patients access services across the health system and coordinating referrals with referring physicians.

The role emphasizes effective communication, coordination of care, and follow-up through EPIC/Care Connect to ensure a smooth patient experience within an indefinite, full-time schedule.

• Report to the Manager and support physicians and patients in assigned care areas.
• Serve as liaison between patients and providers and facilitate service access.
• Coordinate referrals and communicate with referring physicians.
• Provide follow-up via telephone and EPIC/Care Connect to ensure a seamless patient experience.
