Conversio Health (formerly Pacific Coast Pharmacy) is seeking a Patient Care Coordinator to join our Customer Service team in California. This full-time role focuses on telephonic outreach and intake, coordinating with patients, physicians, and staff to ensure accurate information, timely prescriptions, and HIPAA-compliant care.

The ideal candidate has telephonic customer service or sales experience, strong computer skills, and a proactive, confidential approach.

Qualifications

• Minimum 1 year telephonic customer service and/or telephonic sales experience.
• Proficient in Word, Excel and Outlook and able to learn customized software for electronic patient charts.
• Self-starter with strong interpersonal and communication skills for customer service and sales.
• Professional demeanor with attention to detail, organization and planning.

Responsibilities

• Attains familiarity with DME products and procedures to maintain competency and compliance.
• Provide outreach to potential patients, verify information and complete intake form.
• Act as liaison between patients, representatives, and physicians to request prescriptions.
• Ensure compliance with Local, State and Federal laws.
• Observe HIPAA policy regarding patient and co-worker confidentiality.
• Maintain attendance and professionalism in all areas.
