Job summary

The Pines Home Health and Hospice in Prescott, AZ is seeking a Patient Care Coordinator to build relationships with physicians, hospitals, and referral sources, guiding families toward compassionate home health and hospice services.

The ideal candidate has a Bachelor’s degree in Marketing, Business Administration, or related field and at least 3 years of healthcare marketing experience, with home health or hospice background preferred; strong communication and organizational skills are essential.

Qualifications

• Minimum 3 years of healthcare marketing experience; home health and/or hospice experience strongly preferred.
• Strong communication, relationship-building, and public relations skills.
• Ability to work independently with a compassionate, patient-first approach.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources.
• Educate healthcare professionals and community groups about home health and hospice services and benefits.
• Serve as a knowledgeable, compassionate resource for patients and families during the referral and admission process.
• Document outreach activities and maintain accurate records of interactions.
• Ensure all messaging reflects our commitment to dignity, comfort, and quality care.
• Collaborate with care coordination teams to support smooth transitions for patients.
• Monitor community needs and identify opportunities to improve access to home health and hospice care.
