Job summary

The Pines Home Health and Hospice in Arizona is seeking a Patient Care Coordinator to serve as a trusted resource in the community, build relationships with healthcare providers, and guide families toward compassionate home health and hospice services.

You will develop relationships with referral sources, educate professionals and groups about services, and support patients and families through referral and admission, while documenting outreach activities and ensuring messages reflect dignity,

Qualifications

  • Bachelor's degree in Marketing, Business Administration, or related field is preferred.
  • Minimum 3 years of healthcare marketing experience; home health or hospice experience is strongly preferred.
  • Strong communication, relationship-building, and public relations skills.
  • Ability to work independently while maintaining a compassionate, patient-first approach.
  • Demonstrated organizational skills and ability to meet outreach goals.
