Job summary

The Pines Home Health and Hospice is seeking a dedicated Hospice RN to serve as a trusted Case Manager, delivering in-home care that supports patients and families with compassion and dignity.

You will conduct assessments, develop care plans, and coordinate with an interdisciplinary team to ensure comfort and high-quality care in home and community settings.

Qualifications

• Active Arizona RN license required.
• Minimum of 2 years nursing experience; hospice/home health preferred.
• Homecare Homebase (HCHB) experience preferred.
• Valid driver’s license with reliable vehicle.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
• Collaborate with an interdisciplinary team committed to excellence.
• Participate in on-call rotation as needed.

What You’ll Do

What We’re Looking For

• Current RN license in the State of Arizona
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver's license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
• Ability to work independently and a passion for making a real difference
