Job summary

The Pines Home Health and Hospice is seeking a dedicated Hospice Registered Nurse to serve as a Case Manager, delivering compassionate in-home care to patients in the Prescott, AZ area.

You will assess patient needs, develop care plans, administer medications, and educate families while coordinating with an interdisciplinary team to ensure comfort and quality of life.

Qualifications

  • Current RN license in the State of Arizona.
  • Minimum of 2 years of nursing experience, hospice or home health experience preferred.
  • Driver’s license and reliable, insured vehicle.
  • Strong clinical skills and a passion for patient-centered care.
  • Ability to work independently and a passion for making a real difference.

Responsibilities

  • Make a meaningful impact every day as a Hospice RN Case Manager.
  • Deliver care that supports patients in their homes and communities.
  • Guide families through care decisions while providing comfort and dignity.

  • Provide skilled nursing care, administer medications, and manage symptoms.

  • Maintain accurate clinical documentation and communicate changes to physicians.
  • Participate in interdisciplinary team meetings and quality improvement initiatives.
  • Provide emotional support and counseling to patients and families.
  • Supervise hospice aides and delegate responsibilities appropriately.
  • Collaborate with an interdisciplinary team committed to excellence.
  • Participate in on-call rotation as needed.

What You’ll Do

What We’re Looking For

Current RN license in the State of Arizona

  • Minimum of 2 years of nursing experience, hospice or home health experience preferred
  • Homecare Homebase (HCHB) experience preferred, not required
  • Valid driver’s license and reliable, insured vehicle
  • Strong clinical skills and a passion for patient-centered care
  • Ability to work independently and a passion for making a real difference
