• Serve as a knowledgeable resource for patients and families during referral and admission processes.
• Document outreach activities and maintain accurate records.
• Ensure all messaging reflects dignity, comfort, and quality care.
• Collaborate with care coordination teams to support smooth transitions for patients.
• Monitor community needs and identify opportunities to improve access to home health and hospice care.
