CenterWell Home Health in New York is seeking a Patient Services Coordinator to schedule visits and communicate with field staff, patients and physicians to maintain continuity of care. The role also supports day-to-day office and staff management.

Daily duties include managing patient schedules, coordinating add-ons, generating weekly reports, and assisting with on-call rotations as needed. Infection control practices and documentation are part of the role.

Qualificações

• Must possess excellent communication skills and the ability to interact with a diverse group.
• Must have at least 1 year of home health experience.
• Packet review / QI experience preferred.
• Coding certification is preferred.

Responsabilidades

• Schedules visits for patients and manages edits to ensure proper coverage.
• Maintains hospitalization or coordination logs and forwards information to stakeholders.
• Supports weekly scheduling, reports production, and assists branch leadership.
• Assists with on-call rotation and clinical tasks as needed.
• Verifies and routes lab reports and infection control documents when applicable.
