Job summary

CenterWell Home Health is seeking a Patient Services Coordinator to schedule visits, coordinate care between field staff, patients and physicians, and support day-to-day office operations.

Responsibilities include managing patient schedules, handling infection control forms, and ensuring proper staffing for nurse and injection visits, with occasional on-call responsibilities. Strong communication and home health experience are required.

Qualifications

• Must possess excellent communication skills and the ability to interact with a diverse group.
• Must have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Manages schedules for all patients and updates agent availability in the system.
• Initiates infection control forms and handles related reports.
• Provides front-line coverage during lunch and busy periods and supports case conferences.
• Maintains hospitalization log and coordinates notes with staff and physicians.
• Schedules visits, add-ons, and ensures proper staffing for nurse and injection visits.
• Generates scheduling reports and prepares weekly agent schedules for approval.
