Job summary

CenterWell Home Health is seeking a Patient Services Coordinator to manage patient schedules and coordinate care with field staff, physicians, and office teams. The role supports infection control reporting, case conferences, and timely reassignment of patients when staff are unavailable.

Responsibilities include maintaining logs, scheduling visits for skilled nurse and injection visits, and processing add-ons.

Qualifications

• Must possess excellent communication skills and the ability to interact with a diverse group.
• Strong organizational skills and the ability to manage multiple assignments.
• At least 1 year of home health experience.
• Packet review / QI experience preferred.
• Coding certification is preferred.

Responsibilities

• Manage schedules for all patients and reassign as needed.
• Edit schedules when agents call in sick and update worker console.
• Support field staff during lunch hours and busy times; refer clinical questions to Branch Director.
• Maintain client hospitalization log and coordinate notes sharing.
• Ensure visits for skilled nurse/injection services are scheduled appropriately.
• Prepare weekly agent schedules and review for productivity/geography issues.
