Job summary

Mosaic Life Care is hiring an Access Representative to support patient care delivery. The role handles registration, data entry, appointment coordination, and information releases while maintaining confidentiality in a fast-paced clinic setting.

The position requires a high school diploma or GED; an associate degree or certificate is preferred. Travel between facilities is required as part of responsibilities.

Qualifications

• High school diploma or GED required; associate degree preferred.
• Experience with computer data entry, customer service, and medical records is preferred.
• Knowledge of medical terminology is a plus.

Responsibilities

• Schedules, reschedules, and cancels patient appointments and checks patients in/out.
• Enters patient data and creates visits in the system accurately.
• Communicates with patients regarding forms, releases, and documentation.
• Prepares release forms and coordinates information from other providers.
• Answers phones and acts as receptionist for patients and staff.
