Job summary

NOAH is seeking a Lead Patient Navigator in Phoenix, AZ to guide and support a team of patient navigators within the Patient Contact Center. You will oversee referral management, scheduling coordination, and outreach to improve timely access to care.

The role requires extensive knowledge of patient navigation workflows and HIPAA compliance, with a focus on quality, training, and staff development under leadership guidance. Bilingual English/Spanish is preferred.

Qualifications

• Two or more years of experience in patient navigation, referrals, or patient access.
• Experience in an FQHC referral setting preferred.
• Bilingual English/Spanish preferred.
• Must pass pre-employment background check and drug screening.

Responsibilities

• Provide workflow support, peer guidance, and nesting assistance to agents with leadership collaboration.
• Assist with incoming and internal referral management, referral entry, tracking, and scheduling.
• Support patient outreach, documentation accuracy, and follow-up activities.
• Monitor workload, referral tracking, and appointment completion; escalate issues as needed.
