Job summary

NOAH Administration in Phoenix, AZ, seeks a Lead Patient Navigator to provide day-to-day guidance, expert support for patient navigation workflows, and assistance with referrals and scheduling. You will help maintain documentation standards, ensure timely outreaches, and support staff development in collaboration with leadership.

Responsibilities include monitoring work queues, solving patient access barriers, and promoting HIPAA compliance while partnering with the Patient Access Trainer.

Qualifications

• Two or more years of experience in patient navigation, referrals, patient access, scheduling, customer service, call center, or related healthcare operations required.
• Experience in an FQHC referral setting preferred.

Responsibilities

• Provide day-to-day guidance to assigned agents.
• Serve as SME for patient navigation workflows, referrals, outreach, scheduling, documentation, and referral tracking.
• Support onboarding and staff development by enforcing workflows and nesting with leadership.
• Review workload, monitor queues, and assist with staff questions.
• Identify workflow gaps and barriers; elevate recommendations to leadership.
• Promote adherence to HIPAA, privacy requirements, and compliance standards.
• Attend required meetings and trainings; perform other related duties as assigned.
