Job summary

Vynca Inc. is seeking a Patient Access Coordinator to be the front-line contact for patients, families, providers, and health plans.

The role owns inbound calls and end-to-end direct referrals from intake through prep, then delivers fully prepped referrals to EMA for outreach. The position requires healthcare experience, bilingual English/Spanish capabilities, and proficiency with Google Workspace, Salesforce, telephony, and EHR systems.

Qualifications

• Associate degree or medical/business office training preferred.
• Healthcare experience required.
• Experience in palliative care, home health, hospice, discharge planning, or safety-net hospital settings preferred.
• Bilingual English/Spanish required.
• Strong multitasking across phone, documentation, and multiple systems (Google Workspace, Salesforce, telephony, EHR).

Responsibilities

• Serve as primary point of contact for inbound calls and manage call-answer targets.
• Own direct referrals end-to-end from intake to prep and hand-off to EMA for outreach.
• Document call and intake details in real time; maintain databases and logs.
• Follow scripts and quality standards; participate in coaching and team huddles.
• Assist with scheduling, pre-admission assessment, and documentation during intake.
