Job summary

Vynca Inc. is seeking a Patient Access Coordinator to be the first point of contact for patients, families, providers, and health plans. The role manages inbound calls and end-to-end referral intake—from pre-admission tasks to handing off prepared referrals to EMA for outreach.

Healthcare experience is required; bilingual English/Spanish is mandatory. The position involves remote and in-person work with occasional travel and a standard Monday-Friday schedule.

Qualifications

• Healthcare experience required.
• Experience in palliative care, home health, hospice, discharge planning, or safety-net hospital settings preferred.
• Strong communication skills and ability to follow scripts and QA standards.
• Bilingual English/Spanish required.

Responsibilities

• Handle all inbound calls as the primary contact for patients, families, providers, and health plans.
• Own direct referrals end to end—from intake to scheduling support and handoff to EMA for outreach.
• Document call and intake details accurately in real time; maintain databases and logs.
• Follow scripts and participate in coaching, team meetings, and huddles.
