Responsibilities

• Own all inbound calls as the front line for patients, families, providers, and plans.
• Own direct referrals end to end from intake through pre-admission and scheduling.
• Hand off fully prepped referrals to EMA for direct outreach.
• Document call and intake details in real time; maintain logs and contact lists.
• Follow scripts and quality standards; participate in coaching and team huddles.
