Vynca Inc. is seeking a Patient Access Coordinator to own inbound calls and manage end-to-end referrals from providers, hospitals, and community partners.

You will handle intake, pre-admission tasks, scheduling support, and accurate documentation, then pass prepared referrals to EMA for outreach. The role requires healthcare experience and bilingual English/Spanish skills, with comfort using Google Workspace, Salesforce, and various EHR/telephony systems.

Qualifications

• Healthcare experience required.
• Experience in palliative care, home health, hospice, discharge planning, or safety-net hospital settings preferred.
• Ability to recognize clinical trigger words after training; clinical fluency advantageous with SWs, RNs, MAs, chaplains, or scribes.

Responsabilités

• Own inbound calls and act as primary contact with patients, families, providers, and plans.
• Own direct referrals end to end—from intake through scheduling and handoff to EMA for outreach.
• Complete intake for each direct referral: pre-admission assessment, scheduling support, documentation.
• Document call and intake details in real time; maintain databases and contact lists.
• Follow scripts and quality standards; participate in coaching, team meetings, and huddles.
