Job summary

UCLA Health in Rancho Palos Verdes is seeking a dedicated team member to support physicians and patients across primary and specialty care. You will act as a key liaison, helping patients access services throughout the health system and facilitating communication with referring physicians.

You will coordinate referrals and provide follow-up via EPIC/Care Connect to ensure a seamless patient experience, reporting to the Manager and contributing to a coordinated, patient-centered care environment.
