Kids for the Future in Burlington, IA is seeking a Patient Care Coordination Specialist to streamline referrals, patient communication, and prior authorizations, ensuring timely access to care.

The role works with patients, providers, payers and external partners to overcome barriers, maintain accurate EHR documentation, and promote patient safety. Two years medical experience and MA or LPN background preferred.

Qualifications

• Graduate from an approved Medical Assistant training program.
• Graduate from an approved Licensed Practical Nurse training program, is preferred.
• Minimum of two (2) years medical experience or similar experience.
• Certified by the State of Iowa as a Certified Medical Assistant or Licensed Practical Nurse, preferred.
• BLS Certified; Mandatory Reporter for Child and Dependent Adult Abuse.

Responsibilities

• Coordinate referral tracking and ensure timely completion of orders for specialty providers, tests, and services.
• Review EHR tracking reports to follow up on outstanding referral/diagnostic orders and maintain patient safety.
• Educate patients and providers about referral/authorization requirements and language needs.
• Obtain prior authorizations for diagnostic studies, procedures, and services while minimizing delays.
• Document all follow-up actions in the EHR and communicate with providers, payers, and patients.
