Job summary

You will handle referrals and authorizations, coordinate care for preventative services, engage with IPA/HMO payers, and ensure timely communication and documentation while traveling to cover multiple clinics.

Qualifications

• 1+ years experience working with HMO or IPA payers or responsible for care coordination functions.

Responsibilities

• Referrals and Authorizations Coordination: Assist in coordinating referrals and authorizations for specialist care, diagnostics, imaging, etc., ensuring services are covered and the patient’s care continuum is seamless.
• Care coordination: Engage with IPA or HMO payers, members and patients to outreach and schedule appointments for patients due for critical preventative care and facilitate access to necessary services.
• IPA patients utilizing UC service – coordinating care for establishing care in PC.
• Provider coordination and patient engagement.
• Manage provider task messages or boxes, ensuring timely responses, appropriate task delegation, and accurate documentation.
