Job summary

AndHealth LLC in Dayton, Ohio is seeking a seasoned Enrollment & Patient Access Specialist to manage the patient enrollment lifecycle, from referral to program onboarding, in partner practices and EHR systems.

You will identify CDM-eligible patients, coordinate with providers, train staff on referral workflows, and monitor enrollment funnels while maintaining accurate records and regular status updates. Travel to partner practices is expected.

Qualifications

• 5+ years in a field reimbursement, patient access, or clinical operations role.
• Bachelor’s Degree required.
• Experience with provider offices and EHR systems is preferred.

Responsibilities

• Manage patient enrollment from referral to program onboarding.
• Identify CDM-eligible patients in target specialties within partner practices and EHRs.
• Queue patients for provider review and facilitate enrollment.
• Contact patients to support enrollment and transitions.
• Track enrollment funnel and surface process gaps with solutions.
• Serve as on-site expert for medication access challenges.
• Build relationships with practice staff and provide white-glove referral service.
