AndHealth LLC is seeking a Patient & Provider Success Liaison to drive patient identification, enrollment, and seamless provider partnerships. This role is largely on-site at partner practices, often in their EHR environments, and requires proactive outreach to ensure access to care for complex patient populations.

You will build relationships with practice staff, coordinate referrals, and track enrollment through the funnel while maintaining accurate records and working with AI-enabled tools as

Qualifications

• Bachelor’s Degree required.
• Minimum 5 years in reimbursement, patient access, or clinical operations.
• Experience collaborating with physicians, staff, and leadership.

Responsabilités

• Identify eligible patients in partner practices and enroll them in programs.
• Queue patients for provider review and referrals, reducing practice burden.
• Provide white-glove service for referrals and coordinate care with staff.
• Maintain accurate patient records in AndHealth systems and report enrollment metrics.
• Travel regularly to partner practices and work on-site in offices.
