Job summary

The Salvation Army USA Western Territory in Chandler, AZ is seeking a Full Time POH Case Manager to interview clients for emergency assistance, assess needs, and coordinate referrals to housing, health benefits, and community services. You will manage cases, maintain confidential files, and work with diverse populations, including families with children.

A valid AZ driver's license and background check are required.

Qualifications

• Bachelor's degree in social work or related discipline.
• Eight years' experience working with clients in crisis.
• Experience with households including children under 18.
• Extensive knowledge of community resources.
• Proficient with Microsoft Word, Excel, PowerPoint and Publisher; email, Adobe Acrobat, internet, WellSky and HMIS.
• Excellent interpersonal, telephone and written/verbal communication skills.
• Strong detail, organization and file management abilities.
• Ability to maintain confidentiality and work independently or in a team.
• Valid Arizona driver's license with clean driving record and transportation.
• Pass criminal background check and TSA Fleet safety training.

Responsibilities

• Interview clients by phone and in person to determine eligibility for emergency assistance.
• Refer clients to appropriate community agencies and services.
• Develop individual action plans and set goals for each client.
• Provide ongoing case management/assessments at 30, 60, 90 and 365 days.
• Provide referrals for housing, health benefits, financial support and other services.
• Network with outside agencies and community partners to serve clients.
• Prepare monthly statistics for the POH program.
• Maintain positive working relationships with Salvation Army staff, volunteers and partner agencies.
• Coordinate with the Divisional Pathway of Hope Coordinator.
• Assist landlords with rental assistance information collection.
• Maintain client confidentiality across duties and reports.
• Create and update client files in WellSky/HMIS; ensure records are accurate for follow-up.
