Job summary

The Salvation Army Independence Corp is seeking an experienced Case Manager for the Pathway of Hope program. You will coordinate social services, assess eligibility, and provide direct assistance or referrals to families with minor children.

Responsibilities include intake, case planning, home visits, and collaboration with staff to ensure housing stability and financial resources. Strong communication and bilingual abilities are essential.

Qualifications

• Education: Degree in human service area; bachelor’s preferred.
• Minimum of two years of case management experience.
• Ability to work with diverse populations in a sensitive manner.
• Spanish and English language abilities required.

Responsibilities

• Coordinate POH services and assess eligibility for families with minor children.
• Perform intake, develop case plans, and conduct 72-hour assessments.
• Schedule meetings, conduct home visits, and provide referrals as needed.
• Maintain case files and document progress, outcomes, and discharge plans.
• Collaborate with staff, manage data in SIMS, and report service statistics.
