The Salvation Army Blue Valley Corps in Kansas City, MO is seeking a Full-time Case Manager for the Pathway of Hope program. You will coordinate social services, assess families with minor children, deliver strength-based case management, and connect participants to housing stability, increased income, and life skills.

You’ll engage community partners, conduct intake and assessments within 72 hours, develop individualized case plans within a week, provide referrals, advocacy, and home visits,

Qualifications

• Experience delivering strength-based case management in social services.
• Ability to work with diverse families and provide culturally sensitive support.
• Strong communication and collaboration with community partners.

Responsabilités

• Coordinate POH services for families with minor children.
• Conduct intake and 72-hour assessment for new participants.
• Develop and update individualized case plans with goals for housing stability and income.
• Provide referrals and advocacy to access community resources.
• Conduct home visits and schedule regular meetings to review progress.
• Maintain confidential case records and report on outcomes.
