Job summary

The Salvation Army North & Central Illinois Division in Kansas seeks a dedicated professional to coordinate Pathway of Hope (POH) services. You will perform intake, assess families with minor children, and develop strength-based case plans to help participants achieve housing stability, increased income, and self-determination.

Qualifications

• Education: Degree in human service area; prefer bachelor’s degree from an accredited college or university.
• Experience: Minimum of two years of case management experience in a comparable social service program.

Responsibilities

• Coordinate POH services and perform intake and assessment within 72 hours.
• Develop a case plan with participants addressing housing stability, income, and self-determination.
• Maintain case notes, discharge plans, and program documentation in SIMS.
• Provide referrals, advocacy, and home visits as needed.
• Attend agency/community meetings and collaborate with staff and external partners.
