Job summary

The Salvation Army Ferguson Community Empowerment Center is seeking a full-time PoH/ESS Case Manager. The role provides intake, assessment, and ESS services and oversees Pathway of Hope clients, including goal setting and follow-up case management.

The incumbent will work with community agencies, manage PoH client goals, and ensure timely documentation and reporting while maintaining confidentiality and compliance with organizational and government policies.

Qualifications

• Bachelor’s degree in human services or related field with 1 year of experience.
• Strong communication skills and ability to work with diverse cultures.
• Reliable transportation, valid driver’s license, and willingness to pass TSA MVR check.

Responsibilities

• Provide intake, assessment, and ESS services for Pathway of Hope participants.
• Manage PoH client goals and provide ongoing case management.
• Distribute food, vouchers, and checks to clients and refer to other agencies as needed.
• Maintain client confidentiality and accurate documentation in records and SIMS.
• Coordinate with community agencies to maximize services and resources.
• Record data and generate timely statistics and reports.
• Supervise volunteers in food pantry operations.
• Assist with emergency social services during disasters as required.
