Job summary

Qualifications

• Minimum of associate degree in Human Services or related field.
• Background check required and review of results.

Responsibilities

• Schedule and conduct intake appointments for case management.
• Use strengths-based, client-centered case management to identify goals and barriers.
• Implement Pathway of Hope Program Model to support client goals.
• Maintain referrals list for Hamilton County and nearby areas.
• Complete required documentation in SIMS and reporting per policies and funder standards.
• Network with social services providers and represent The Salvation Army at events.
• Make budgetary recommendations to senior social services leaders.
• Attend divisional trainings and participate in coordinating meetings.
