Job summary

The Salvation Army Independence Corps seeks a Case Manager for Pathway of Hope. This full-time role coordinates social services, conducts intake, and develops case plans to help families with minor children achieve housing stability and greater self-sufficiency.

Responsibilities include outreach, eligibility screening, providing referrals, documenting progress, coordinating with staff and community partners, and maintaining client confidentiality.

Qualifications

• Education: Degree in human services or related field; 2+ years case management experience preferred.
• Experience: Strength-based case management and work with families with minor children.
• Skills/Abilities: Bilingual English/Spanish preferred; strong communication and organizational skills.

Responsibilities

• Outreach and engagement with community partners.
• Conduct intake and eligibility screening.
• Develop case plans and monitor progress with participants.
• Provide referrals and coordinate services with staff and external providers.
• Maintain case files and ensure documentation is complete and confidential.
