Job summary

The Salvation Army North & Central Illinois Division is seeking a dedicated professional to coordinate Pathway of Hope services. You will perform intake, assess participants, and develop case plans focused on housing stability, income growth, and self-determination, while collaborating with corps staff and the POH program manager.

This full-time role requires strong outreach, English/Spanish communication, and the ability to manage documentation and reporting in SIMS and related systems.

Qualifications

• Degree in human service field; bachelor's degree preferred.
• Minimum of two years of case management experience in a social service program.

Responsibilities

• Coordinate Pathway of Hope (POH) case management with staff.
• Perform intake with required documentation within 72 hours of contact.
• Develop a case plan with goals for housing stability and financial resources.
• Provide referrals and information to connect participants with community resources.
• Maintain case files with assessments, plans, progress notes and discharge plan.
