Job summary

Haven Home Care is expanding services into Eastern Ohio and seeks a Supervisory LPN for our growing team. This role focuses on open/supervisory visits, documentation, and pre-employment testing for aides.

It is a per-visit position with no hourly wage and mileage reimbursement for travel. The successful candidate will manage visits across Ashtabula, Columbiana, Mahoning, and Trumbull counties, support client growth, and ensure regulatory compliance.

Qualifications

• Must be at least 18 years old.
• Graduation from an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license and reliable transportation with work coverage.
• Current CPR certification.
• Successful completion of criminal background check and tests.
• Residence in Trumbull or Mahoning County preferred.

Responsibilities

• Complete open visits and supervise Home Health Aides every 60 days.
• Collect documentation and maintain agency records per regulatory standards.
• Conduct pre-employment testing for Home Health Aide candidates.
