Job summary

Haven Home Care Inc. is expanding into Eastern Ohio and seeks an LPN for a supervisory, per-visit role. You will complete open visits, collect documentation, and supervise Home Health Aides without delivering skilled nursing care.

Travel to Ashtabula, Columbiana, Mahoning and Trumbull counties is required with mileage reimbursed. The position offers flexible scheduling and growth as the client base expands.

Qualifications

• Must be at least 18 years old.
• Graduate of an accredited School of Practical Nursing.
• Current Ohio LPN license in good standing.
• Valid driver's license, reliable transportation, and work coverage insurance.
• Current CPR certification.
• Successful background check and tests.
• Residence preferred in Trumbull or Mahoning counties.

Responsibilities

• Prepare accurate documentation per agency standards and regulatory rules.
• Conduct supervisory visits every 60 days for Home Health Aides and open visits for new clients.
• Perform pre-employment testing (skills and written tests) for aides.
• Travel to client homes across Ashtabula, Columbiana, Mahoning and Trumbull counties.
