Job summary

21st Century Home Health Services in Vacaville is seeking a Speech Therapist to provide in-home care across Solano County, partnering with a Case Manager to evaluate and treat speech, language, cognitive, and swallowing disorders.

You will educate patients, advocate for quality care, and collaborate with an interdisciplinary team using company tools and flexible scheduling in a fully in-person role.

Qualifications

• Current unrestricted CA Speech Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience in home health care and with high acuity patients is beneficial.
• Experience working with an interdisciplinary team is highly desired.

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive, and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.
