21st Century Home Health Services in Vacaville is seeking a Speech Therapist to provide in-home care across Solano County, partnering with a Case Manager to evaluate and treat speech, language, cognitive, and swallowing disorders.

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive, and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.
Prior to scheduling an in‑person interview, all Candidates are required to complete a computer literacy assessment. This is to ensure you are set up for success and can navigate in our environment including electronic charting and use of our web‑based communication application. The results of the assessment are confidential and will be used as one of multiple data points that go into the hiring decision process.
