Job summary

21st Century Home Health Services, a leading home health provider, seeks a Part Time Physical Therapist Case Manager to evaluate and address patient needs in their homes across Contra Costa County, including Richmond. You will restore function, minimize disability, and help patients achieve maximum performance within their abilities.

You will collaborate with an interdisciplinary team, supervise PTAs, and ensure high-quality care while navigating regulatory guidelines and therapy plans.

Qualifications

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience with an interdisciplinary team is highly desired.
• Case Management fundamentals are highly desired.

Responsibilities

• Provide skilled evaluations in the patient’s home and develop care plans with PTAs.
• Help patients regain mobility and independence.
• Coordinate with physicians to ensure quality care.
• Communicate patient progress to MD and adjust plans as needed.
• Assist with documentation and administrative tasks and ensure compliance.
