Job summary

21st Century Home Health Services is seeking a Part Time Physical Therapist Case Manager to evaluate and address the needs of individuals in their homes within Santa Cruz County. You will develop care plans, restore function, and work with PTAs to deliver evidence-based home health care.

Applicants should hold a CA Physical Therapist License, BLS/CPR, and a valid CA driver's license. Flexible schedules and strong interdisciplinary collaboration are emphasized to improve patient outcomes.

Qualifications

• Current unrestricted CA Physical Therapist License is required.
• BLS/CPR Certification required.
• Valid CA Driver's License required.
• Experience in home health care is highly preferred.
• Knowledge of local community resources is required.

Responsibilities

• Evaluate patients in their homes and create care plans with PTAs as per regulatory standards.
• Restore function and maximize client independence through prescribed PT programs.
• Coordinate with patients' physicians to ensure quality care.
• Communicate progress and outcomes to MDs and the case team.
• Supervise PTAs where applicable and manage case load.
